Except it's often a simple textual replace of mom with landlady, etc. Landlady is an easy fix, and it's good enough. These games often take a long time to get updates out; adding complex text changes makes it more likely to accidentally let something prohibited slip in, and takes more time.

Fair point on it being obvious and people figuring it out, but not everyone wants to use an incest patch as they have zero interest in incest, so therefore you can potentially lose some people due to it, as it is off-putting.
